Research published in Endocrine Reviews from the University of Oxford confirmed that estrogen deficiency directly results in hepatic insulin resistance and impaired fat metabolism in the liver, establishing the liver as a central player in women's hormonal health, not a peripheral one.
Estrogen Dominance and the Liver Connection
Estrogen dominance, a state in which estrogen is high relative to progesterone, is one of the most common hormonal complaints in women in their 30s and 40s. Most conversations around estrogen dominance focus on the ovaries or the adrenals. Very few point to where estrogen is actually cleared: the liver.
Circulating estrogens are conjugated in the liver, reducing their reactivity, and then excreted into bile, where, through enterohepatic circulation, they reach the small intestine. Rather than being excreted, gut microbial enzymes called beta-glucuronidases can deconjugate the conjugated estrogens back into active parent estrogens that are reabsorbed into circulation. This means that even when the liver does its job well, an imbalanced gut microbiome can undo that work by recirculating estrogen that was supposed to leave the body. Research published in the International Journal of Cancer in 2025 confirmed that disruptions in this estrobolome pathway directly affect systemic estrogen levels and downstream hormonal health. The liver and gut are a team. When either one is compromised, estrogen accumulates.
What Cortisol Does to the Liver (And What the Liver Does With Cortisol)
Chronic stress does not just affect your mood and sleep. It creates a sustained demand on the liver to process and clear elevated cortisol. Chronic cortisol elevation is directly associated with central fat accumulation, insulin resistance, and metabolic dysfunction, all of which place additional burden on the liver to process excess glucose and fat simultaneously.
A 2024 randomized clinical trial published in PMC found that elevated cortisol is a stress-related hormone that contributes directly to hepatic fat accumulation and insulin resistance, and that lifestyle modifications targeting cortisol measurably improved liver steatosis in participants. Research also found that postmenopausal women have increased activity of the enzyme responsible for reactivating cortisol in liver tissue and adipose tissue, which may contribute to the metabolic dysfunctions associated with menopause and aging in women. In other words, as women age and hormonal shifts accumulate, the liver's cortisol burden increases, and the downstream metabolic consequences compound.
The Signs Your Liver Is Struggling (That Nobody Connects to the Liver)
A struggling liver rarely announces itself with obvious symptoms. It does not hurt. It does not swell visibly. The liver is a women's health issue. It is just not being framed that way yet.
Helpful Tips
- Eat cruciferous vegetables daily. Broccoli, cauliflower, Brussels sprouts, and kale contain compounds called glucosinolates that support phase two liver detoxification, the process by which the liver packages hormones for elimination.
- Reduce alcohol. The liver prioritizes alcohol metabolism above all other functions. Every time it is processing alcohol, it is deprioritizing estrogen and cortisol clearance.
- Support your gut microbiome actively. The estrobolome, the community of gut bacteria responsible for estrogen metabolism, directly determines how much estrogen gets recirculated versus eliminated. Fermented foods, prebiotic fiber, and minimizing antibiotic use all support this system.
